If you require any vaccinations relating to foreign travel you will need to complete a Travel Vaccination form from reception. We then ask you to contact the surgery 48-72 hours after handing in the form and the reception team will advise you which vaccines are required as per the nurses. We will then proceed to book you an appointment with the Nurse for your vaccinations. We ask all patients to try and hand their vaccination forms in 4-6 weeks before travelling to allow time for the Nurses and for booking an appointment.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please fill out the online form then contact the surgery after 10am three working days after completion. We will then be able to let you know if you require any vaccinations and offer you an appointment with the nurse.
