New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill, bank statement, tenancy agreement or any official document with your name and address on. We also require one form of photographic ID, if you do not have a form of photographic ID then we will also accept a birth certificate.
You will need to complete a registration form (GMS1) and also a New Patient Screening Questionnaire. You can collect these from the surgery or alternatively you can print the below documents, complete them at home and then bring the completed forms to the surgery.
Registration forms are accepted at reception Monday-Friday between the hours of 1pm and 5pm.
For patients registering with Issa Medical Centre: Please now note that due to us offering an essential Covid-19 vaccine service at the practice, we are having to streamline administration tasks. Patients can collect registration forms any weekday between the hours of 8:30-18:00 and we will ask that once you have completed your forms to contact the surgery on 01772 759330 and you will be advised which day you can return the forms to us as it will need to be on a day when the practice is not administering the Covid-19 vaccine.
We offer all of our patients a new patient screening appointment, if you are a patient who has no medical conditions we will offer you an appointment with a non clinical member of staff on a Saturday. Any patients with long standing conditions (such as Diabetes, Hypertension, COPD etc.) require a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
If you do take any repeat medication please bring a copy of your medication list from your previous practice so that staff are able to add these onto your records. If you are unable to obtain a copy then please bring the boxes of your medication and we may be able to use these as long as your details are labeled on the box.
